Scope of Appointment Form (SOA)

  In-Home Appointments:
The agent must have the Scope of Appointment form signed BEFORE meeting with the client at their home.

  In-Office Appointments:
The agent may have the Scope of Appointment form signed at his/her office when the potential client arrives for the meeting.

Make sure that you check all of the boxes on the SOA form so that you are able to advise the potential enrollee on all options, from Medicare Supplement to Medicare Advantage to Prescription Drug Plans and everything in between.
